Hematology lab techs: Send the following to reviewing pathologist:

1.) Prefer 1 unstained slide and 1 stained slide. Minimum, 1 stained slide. Label the slides using pencil, indelible ink or stain resistant labels with patient name, at least one additional patient identifier (SQ accession number is preferred), and collection date
2.) CBC reported results (i.e., Sunquest IRA report), or if the CBC or diff is not reported, send the instrument printout with technologist’s suggested differential
3.) Sunquest IRA report of pathology review order with attached LIS sticker (TRPBS)
4.) Completed Tech Requested Pathology Review PBS form, HE0158-F1
