Formalin fix (10% neutral buffered formalin) and paraffin embed tissue
Protect from excessive heat
Transport tissue block or 5 unstained 5-micron slides (minimum: 3 slides) for extraction
Tissue block will be refiled for storage after testing
Slides will be destroyed during extraction process and will not be returned
